IACUC forms
Tips:
- The online forms work best with web browsers Google Chrome and Firefox (make sure they are updated)
- It is recommended to allow pop-ups from specifically https://myresearch.wsu.edu (manage in browser settings)
- Issues with forms on recommended browsers? Try pressing “ctrl” and “F5” simultaneously to refresh the browser’s cache
- Trouble downloading forms? Use Microsoft Office for Word and Excel documents and Adobe Acrobat DC for PDF documents. Ask your department’s IT or Animal Welfare Program office for help.
Online Animal Subject Approval Form (ASAF)
Required for all new submissions and 3-year resubmissions. All first-time submissions in the online form will require IACUC review, as there are new questions and sections not covered in the previously used PDF forms.**To access all online form protocols, log into MyResearch (http://myresearch.wsu.edu), then hover the mouse over "Compliance", then hover the mouse over "IACUC", and then click on the tab labeled "MyProtocols".
